Summary
The Neodeluxe Sidebar: USClaims option allows users to submit funding requests directly to the USClaims system using the interface detailed below.

Neodeluxe Sidebar: USClaims Option Interface
The Neodeluxe Sidebar contains the following elements:

Request Information section: This section contains the following fields: Amount Needed, Is this a Subsequent Request?, Plaintiff Name*, Plaintiff Email*,and Plaintiff Phone. Plaintiff Address * section: This section contains the following fields: Street 1*, Street 2, City*, State*, Postal/Zip Code*, Plaintiff DOB*, Plaintiff SSN, What Type of Case is this?, Other Case Type, State of Claim*, Date of Accident*, Court Docket Number, Court Name, Defendant Name, Insurance Limits, Insurance Carrier Name, Accident Summary, Intake Estimate, Attorney's Name*, Attorney's Email, and Attorney's Phone Number. Attorney’s Address * section: This section contains the following fields: Law Firm, Paralegal, Paralegal Email, Other Funder Name, Estimated Other Funder Payoff, and Attach Documents.

Submit a Request
Follow these steps to submit a funding request:
- Navigate to the Filevine project you want to manage in your browser tab and select USClaims in the Neodeluxe Sidebar Active Tools. The USClaims form will be displayed.
- Click Autofill Form to automatically retrieve the available data in the form from the mapped fields. Users can also enter and edit the form’s information manually.
- Ensure all the mandatory fields in the form are filled denoted by an *.
- If the field is not being mapped, please contact the admin (It could require an override configuration).
- All the data submitted in the Neodeluxe Sidebar: USClaims option will NOT be updated on the Filevine Project.
- Click Submit Request to send the funding request to the USClaims team.
- If one or more mandatory fields are empty when the user clicks Submit Request the message: You are missing required fields is displayed.
- When all the mandatory fields are filled, and the user clicks Submit Request, the following messages are displayed:
- Your funding request submission is in progress.
- Your funding request has been submitted successfully.
Once the Funding Request is submitted, the USClaims team will manage your request, therefore all support related queries should be directed towards the USClaims team.
